LEG ECZEMA & STASIS DERMATITIS
OVERVIEW
Leg eczema, also known as stasis dermatitis or venous eczema, is an inflammatory skin condition associated with chronic venous insufficiency and chronic venous hypertension.
Unlike many other forms of eczema, leg eczema may be a visible manifestation of an underlying circulation problem. Increased venous pressure in the lower legs produces chronic inflammation within the skin and surrounding tissues, contributing to itching, scaling, redness, discoloration, and progressive skin changes.
Patients are often surprised to learn that a persistent or recurrent rash around the ankles or lower legs may be related to their veins. The dermatitis may temporarily improve with topical treatment while the underlying venous hypertension continues to drive inflammation.

WHAT HAPPENS TO THE SKIN?
Chronic venous hypertension affects more than the visible veins. Persistent pressure within the lower-extremity venous circulation can lead to fluid accumulation, inflammation, and changes within the skin and subcutaneous tissues.
Over time, affected skin may become:
- Dry, itchy, or scaling
- Red or chronically inflamed
- Brown, tan, or reddish-brown
- Thickened, firm, or indurated
- Increasingly sensitive and fragile
Brown or rust-colored discoloration commonly seen around the ankles and lower legs is often caused by hemosiderin deposition following leakage and breakdown of red blood cells within tissues exposed to chronic venous hypertension.
In the clinical classification of chronic venous disease, pigmentation and stasis dermatitis are recognized as CEAP C4a skin changes, representing progression beyond uncomplicated varicose veins.
WHY TREATING THE ECZEMA ALONE MAY NOT BE ENOUGH
Topical medications can reduce itching and inflammation, and appropriate skin care can help restore and protect the damaged skin barrier.
However, these treatments do not correct chronic venous hypertension.
When significant venous insufficiency persists beneath the skin, dermatitis may improve temporarily and then return. Recognizing the venous component is therefore important in patients with persistent or recurrent lower-leg eczema.
Repeated scratching can further damage already fragile skin. Excoriations, fissures, and minor trauma may create additional inflammation and provide an entry point for secondary bacterial infection.

STASIS DERMATITIS AND CELLULITIS
Stasis dermatitis can produce redness, warmth, swelling, itching, scaling, and inflammation of the lower legs, findings that may sometimes resemble cellulitis.
The two conditions are not mutually exclusive. Cellulitis can develop in skin already affected by stasis dermatitis, particularly when chronic edema, scratching, fissures, or skin breakdown compromise the normal protective skin barrier.
Secondary infection and worsening inflammation are frequently what bring patients with longstanding venous skin changes for medical evaluation.
WHEN LEG ECZEMA APPEARS TO “SPREAD”
An important complication of significant or persistent stasis dermatitis is autoeczematization, also called an id reaction or autosensitization dermatitis.
In this reaction, intense inflammation originating in the lower legs triggers a secondary eczematous eruption at sites distant from the original dermatitis.
A patient whose primary dermatitis began around the ankles or lower legs may subsequently develop new, intensely itchy patches or small inflammatory lesions on the opposite leg, thighs, arms, trunk, or other areas. The eruption may appear symmetrical or “mirrored,” creating the impression that the original eczema has spread throughout the body.
The distant areas are not developing venous insufficiency. Rather, they represent a secondary inflammatory skin reaction associated with the primary dermatitis.
Recognizing and controlling the original inflammatory process is therefore an important part of management.
HEMOSIDERIN STAINING AND BROWN DISCOLORATION
The characteristic brown, tan, or rust-colored discoloration of the lower legs develops as chronic venous hypertension affects the small vessels and surrounding tissues.
Red blood cells can escape into the tissues and break down, leaving hemosiderin-containing pigment behind. Over time, this produces the familiar brown staining seen around the ankles and lower legs in patients with longstanding chronic venous disease.
Hemosiderin staining may be slow to improve and can persist even after the underlying venous disease has been treated. Treatment of chronic venous insufficiency addresses the abnormal venous circulation contributing to continued inflammation and tissue injury; it does not guarantee reversal of established pigmentation.
PROGRESSION OF VENOUS SKIN DISEASE
Persistent venous hypertension can eventually produce more advanced changes in the skin and underlying tissues.
Chronic inflammation may progress to fibrosis, induration, and lipodermatosclerosis. The skin may become increasingly fragile and susceptible to injury and breakdown.
Repeated scratching, minor trauma, fissuring, and persistent inflammation can further compromise the skin barrier. In advanced chronic venous insufficiency, tissue breakdown can eventually result in a venous leg ulcer.

MEDICAL AND VENOUS EVALUATION
Evaluation begins with examination of the skin and lower extremities, including the distribution and character of the dermatitis, pigmentation, edema, tissue changes, visible veins, and areas of previous or active skin breakdown.
When the clinical pattern suggests underlying venous disease, venous Duplex ultrasound evaluates blood flow, valve function, and the presence and distribution of pathologic venous reflux.

MANAGEMENT OF STASIS DERMATITIS
Management of stasis dermatitis requires attention to both the inflamed skin and the chronic venous hypertension responsible for the condition. Treating the visible eczema alone may provide temporary relief but does not correct the underlying venous disease.
Management may include:
- Properly measured graduated compression stockings to help control venous hypertension and lower-extremity edema
- Topical treatment to control active inflammation and itching, recognizing that improvement may be temporary when underlying venous hypertension persists
- Skin barrier protection and appropriate moisturization to reduce dryness, fissuring, and further irritation
- Treatment of secondary bacterial infection when present
- Venous Duplex ultrasound to identify pathologic venous reflux and determine the extent and distribution of chronic venous insufficiency
- Treatment of underlying chronic venous insufficiency once established, with treatment planning based on venous anatomy, ultrasound findings, and clinical severity
Control of venous hypertension is particularly important because continued inflammation can contribute to progressive skin and tissue changes even after the acute eczema improves.
Evaluate Persistent Leg Eczema or Venous Skin Changes
Persistent or recurrent eczema of the lower legs may represent more than an isolated skin condition. Evaluation can determine whether chronic venous insufficiency and venous hypertension are contributing to the inflammation and skin changes.
