Evaluation of Hair Loss and Hair-Growth Disorders
OVERVIEW
Hair loss may result from genetic, hormonal, autoimmune, inflammatory, medication-related, nutritional, systemic, or traumatic factors. Some conditions cause increased shedding without permanent damage to the hair follicle, while others may progressively reduce hair density or permanently damage follicles.
Hair loss may develop gradually or suddenly, affect the entire scalp or localized areas, and occur with or without visible scalp changes.
Because different disorders can produce similar patterns of thinning or shedding, hair loss is a clinical finding rather than a single diagnosis. Identifying the underlying cause is important before treatment is recommended.
COMMON CAUSES OF HAIR LOSS
Common causes and patterns include:
- Androgenetic Alopecia (Pattern Hair Loss) — Gradual genetically influenced thinning affecting both men and women, although the distribution commonly differs.
- Telogen Effluvium — Increased shedding that may develop following illness, surgery, significant stress, rapid weight change, hormonal changes, medication changes, or other physiologic stressors.
- Alopecia Areata — An autoimmune condition that commonly produces smooth, well-defined areas of hair loss and may involve the scalp, beard, eyebrows, or other hair-bearing areas.
- Inflammatory or Scarring Alopecia — Disorders in which inflammation damages hair follicles and may result in permanent hair loss.
- Traction or Traumatic Hair Loss — Repeated tension, pulling, chemical processing, heat, or other physical injury may damage the hair shaft or follicle.
- Medication-Related Hair Loss — Certain medications may alter the normal hair-growth cycle and contribute to shedding or thinning.
- Systemic, Hormonal, or Nutritional Factors — Thyroid disorders, hormonal changes, iron deficiency, nutritional abnormalities, and other medical conditions may contribute to hair loss.
- Inflammatory or Infectious Scalp Disease — Scalp inflammation or infection may interfere with normal follicular function and contribute to shedding or localized hair loss.
More than one factor may contribute to hair loss in the same patient.
PATTERN HAIR LOSS
Androgenetic alopecia is one of the most common causes of progressive hair thinning.
In men, thinning frequently begins at the frontal hairline, temples, or crown. In women, pattern hair loss more commonly produces diffuse thinning over the central scalp and widening of the part, although other patterns may occur.
Evaluation considers the distribution and progression of hair loss, follicular findings, family history, medical history, and other potential contributing factors.
Because hair growth occurs slowly, treatment of pattern hair loss generally requires long-term management and realistic expectations regarding the time required to assess response.
SUDDEN OR EXCESSIVE HAIR SHEDDING
Not all hair loss represents permanent follicular loss.
Telogen effluvium occurs when an increased number of hair follicles enter the resting and shedding phase of the normal hair cycle. Patients may notice substantially more hair during washing, brushing, or normal daily activity.
Potential contributing factors include:
- Significant illness or fever
- Surgery or physiologic stress
- Emotional stress
- Rapid weight loss or major dietary changes
- Hormonal changes
- Certain medications
- Iron deficiency or other nutritional abnormalities
- Thyroid or other systemic disorders
Because increased shedding may begin weeks or months after the triggering event, the connection is not always immediately apparent.
ALOPECIA AREATA
Alopecia areata is an autoimmune hair-loss disorder in which the immune system targets hair follicles.
It commonly produces smooth, well-defined areas of hair loss, although more extensive patterns may occur. The scalp is frequently affected, but alopecia areata may also involve the beard, eyebrows, eyelashes, or other hair-bearing areas.
Evaluation helps distinguish alopecia areata from fungal infection, traumatic hair loss, inflammatory alopecia, and other causes of localized hair loss.
INFLAMMATORY & SCARRING HAIR LOSS
Some inflammatory scalp disorders can damage the hair follicle itself.
In scarring alopecia, ongoing inflammation may progressively destroy follicles and replace them with scar tissue. Once a follicle has been permanently destroyed, normal hair growth from that follicle cannot be restored.
Findings such as scalp redness, scaling, tenderness, burning, pustules, crusting, loss of follicular openings, or progressive localized hair loss may require additional evaluation.
Early diagnosis is particularly important when a scarring process is suspected because treatment focuses on controlling inflammation and limiting further follicular damage.
SCALP CONDITIONS & HAIR LOSS
Inflammatory and infectious scalp conditions may coexist with hair loss or contribute to temporary shedding and hair breakage.
Conditions such as seborrheic dermatitis, psoriasis, contact dermatitis, folliculitis, and fungal infection can produce itching, scaling, redness, crusting, or inflammation of the scalp.
Persistent scalp disease should be evaluated separately from the underlying pattern of hair loss rather than assuming that all shedding or thinning has the same cause.

MEDICAL EVALUATION OF HAIR LOSS
Evaluation begins with a review of the pattern and duration of hair loss, degree of shedding, scalp symptoms, medical history, medications, family history, hair-care practices, and potential triggering events.
Examination includes assessment of:
- Distribution and pattern of hair loss
- Hair density and hair shafts
- Scalp and follicular openings
- Evidence of redness, scale, inflammation, or scarring
- Other skin or nail findings that may help establish the diagnosis
Additional evaluation may include laboratory testing, fungal or microbiologic testing, dermoscopic examination, or scalp biopsy when clinically indicated.
TREATMENT APPROACH
Treatment is directed toward the underlying cause of hair loss and may include:
- Medical treatment of pattern hair loss
- Control of scalp inflammation
- Treatment of infection when present
- Treatment of autoimmune or inflammatory hair disorders
- Modification of contributing hair-care practices or environmental factors
- Evaluation of medication-related contributors
- Evaluation and correction of relevant systemic, hormonal, or nutritional factors
- Long-term monitoring of chronic or progressive hair-loss disorders
Hair grows slowly, and improvement in the underlying condition may occur well before a visible change in hair density becomes apparent.
