Medical Laser Treatment for Facial Redness, Broken Capillaries & Visible Veins
Facial redness and visible blood vessels may result from a range of vascular and inflammatory skin conditions. Some patients have diffuse redness or flushing, while others are primarily concerned with spider veins, broken capillaries, or larger visible vessels on the face or nose.
These vascular changes can occur at different depths and vessel sizes. Superficial capillaries, diffuse vascular redness, and deeper facial veins do not necessarily respond to the same laser wavelength or treatment parameters.
At International Vein & Skin Institute (IVSI), facial vascular laser treatments are personally performed by Jozef Tryzno, MD, using Class IV medical laser technologies. Treatment is selected according to the vascular pattern, vessel size and depth, underlying skin condition, treatment area, and clinical response.
COMMON FACIAL REDNESS & VASCULAR CONCERNS
Laser treatment may be considered for:
- Facial spider veins
- Broken capillaries (telangiectasias)
- Visible veins on the face and nose
- Diffuse facial redness
- Rosacea-related vascular redness
- Benign cutaneous flushing
- Generalized essential telangiectasia
- Hemangiomas
- Post-acne redness
- Port-wine stains
Some patients have predominantly individual visible vessels, while others have diffuse redness or a combination of vascular changes.
Because these conditions differ in cause, vessel size, depth, distribution, and behavior, treatment must be individualized.
MEDICAL EVALUATION COMES FIRST
Not all facial redness or visible facial veins represent the same vascular process, and not all vessels respond to the same laser wavelength.
Evaluation helps determine:
- The underlying cause and vascular pattern
- Whether treatment is directed primarily at diffuse redness, individual vessels, or both
- The size and depth of visible blood vessels
- Which laser wavelength or combination of wavelengths is most appropriate
- Whether an underlying inflammatory condition requires medical management in addition to vascular laser treatment
LASER TREATMENT OPTIONS FOR FACIAL REDNESS & VISIBLE VEINS
Different vascular patterns require different wavelengths and treatment parameters. At IVSI, Class IV medical vascular lasers are used to treat superficial redness, small broken capillaries, facial spider veins, and selected deeper visible vessels.
Treatment is personally performed by Jozef Tryzno, MD, allowing laser selection and treatment parameters to be adjusted according to the vessel size, depth, location, skin characteristics, and response to treatment.
No single vascular laser is appropriate for every facial vessel or pattern of redness.
PULSED DYE LASER (PDL) — 595 NM
595-nm Pulsed Dye Laser
PDL targets oxyhemoglobin within superficial blood vessels and may be used for:
- Diffuse facial redness
- Rosacea-related vascular redness
- Facial spider veins and broken capillaries
- Small superficial vessels around the nose and cheeks
- Post-acne redness
- Port-wine stains
- Selected superficial vascular lesions
PDL is particularly useful for superficial vascular changes and diffuse redness. Treatment parameters can be adjusted according to the vascular pattern and the desired treatment response.
Because PDL does not target the hair follicle, it can also be used over hair-bearing areas without producing permanent hair reduction.
Nd:YAG LASER — 1064 NM
1064-nm Nd:YAG Laser
Nd:YAG laser penetrates more deeply than PDL and may be used when the vascular concern involves larger or deeper facial blood vessels.
It may be used for:
- Larger or deeper facial veins
- Visible blue or purple vessels
- Nasal and facial veins that extend deeper beneath the skin
- Vascular lesions not adequately addressed with a more superficial wavelength
The 1064-nm wavelength allows treatment of vessels located deeper within the skin, complementing PDL treatment of more superficial vascular changes.
Treatment parameters are selected according to the vessel size, depth, location, skin characteristics, and clinical response.
Because Nd:YAG energy can affect hair follicles, treatment over hair-bearing areas may result in permanent hair reduction. This is considered when selecting the appropriate wavelength and treatment approach.
CYNERGY MULTIPLEX™ — SEQUENTIAL DUAL-WAVELENGTH TREATMENT
PDL 595 nm + Nd:YAG 1064 nm
The Cynergy MultiPlex™ system allows sequential delivery of PDL and Nd:YAG wavelengths during the same treatment session.
This approach may be useful when facial vascular changes involve a combination of superficial and deeper vessels, such as diffuse redness or small spider veins occurring together with larger visible facial veins.
- PDL targets more superficial vascular redness and small vessels
- Nd:YAG reaches larger or deeper vascular structures
- The wavelengths may be used individually or sequentially according to the vascular pattern being treated
Not every patient requires dual-wavelength treatment. The wavelength or combination is selected according to the vessel size, depth, location, and treatment response.
When treating hair-bearing areas, PDL may be used alone to avoid unintended permanent hair reduction from the Nd:YAG wavelength.
EXPECTED RESPONSE AND RECOVERY
Vascular laser treatment requires delivery of sufficient energy to produce a therapeutic effect within the targeted blood vessels. Treatment intensity can be adjusted according to the vascular condition, treatment goals, desired recovery time, and number of treatments planned.
Treatment may be performed using:
- Lower-intensity settings over multiple treatment sessions, generally producing less visible bruising and shorter recovery
- Higher-intensity settings when appropriate, which may produce more pronounced purpura or bruising and a longer recovery period
With Pulsed Dye Laser (PDL), purpura (purple discoloration or bruising) was historically used as a visible treatment endpoint. Current treatment approaches allow energy settings to be adjusted so that significant bruising is not required for effective treatment.
Following vascular laser treatment:
- Temporary redness and mild swelling may occur
- Some degree of purple discoloration or bruising may occur depending on treatment settings
- Recovery may range from a few days to several weeks, depending on treatment intensity, vascular condition, and degree of purpura
- Treatment intensity can be adjusted when minimizing visible bruising or downtime is an important consideration
Tanned skin is a relative contraindication to PDL treatment. Treatment may need to be postponed until the tan has faded to reduce unnecessary interaction between laser energy and increased skin pigment.
WHAT LASER TREATMENT CAN — AND CANNOT — DO
Vascular laser treatment can reduce visible facial blood vessels, spider veins, broken capillaries, and diffuse vascular redness by selectively targeting abnormal or unwanted blood vessels within the skin.
Individual visible vessels may respond very well to treatment, but vascular conditions such as rosacea, flushing disorders, and generalized telangiectasia reflect an underlying tendency to develop vascular changes.
Laser treatment may:
- Reduce or eliminate individual visible facial vessels
- Reduce spider veins and broken capillaries
- Improve diffuse vascular redness
- Reduce persistent post-acne redness
- Improve the overall appearance of vascular skin changes
Laser treatment does not:
- Cure rosacea or flushing disorders
- Eliminate the underlying tendency to develop new telangiectasias or visible vessels
- Prevent new vascular changes from developing over time
Some vessels may require more than one treatment, and new vessels may develop in the future even after previously treated vessels have improved.
