Fine lines and wrinkles develop as part of the natural aging process, as collagen and elastin production gradually decline over time. Early changes often appear as fine lines in the late 20s or 30s and may deepen with continued aging, sun exposure, and environmental factors such as ultraviolet (UV) light or smoking.

While wrinkles are inevitable, their visibility and associated skin texture changes can often be improved with appropriately selected laser-based treatments.

At International Vein & Skin Institute (IVSI), laser skin resurfacing treatments are personally performed by Jozef Tryzno, MD, using Class IV medical laser technologies. Treatment is individualized according to the skin condition, treatment goals, desired treatment depth, and recovery preferences.

WHAT IS LASER SKIN RESURFACING

Laser skin resurfacing uses controlled laser energy to improve skin texture and structural changes associated with aging and sun damage by stimulating collagen remodeling and, with ablative treatments, removing controlled portions of the skin surface.

The goal is to improve fine lines, wrinkles, uneven skin texture, and overall skin quality.

Resurfacing treatments vary in depth and intensity and are selected according to the skin condition, treatment goals, and desired recovery profile.

RESURFACING APPROACHES

Laser treatments for fine lines, wrinkles, and skin texture vary in depth, intensity, and the way they interact with the skin.

Non-ablative skin remodeling – stimulates collagen and elastin remodeling without removing the skin surface
Fractional resurfacing – creates microscopic treatment zones while preserving surrounding skin to support healing
Ablative resurfacing – removes controlled portions of the skin surface while stimulating tissue remodeling

Treatment depth and intensity can be adjusted according to the skin condition, degree of correction desired, treatment response, and acceptable recovery time.

NON-ABLATIVE SKIN REMODELING

PicoSure Focus™ Laser

Non-ablative skin remodeling may be used for early fine lines, uneven skin texture, and early age-related skin changes.

Delivers ultra-short picosecond laser pulses into deeper skin layers
Stimulates collagen and elastin remodeling
Preserves the skin surface while minimizing interruption to daily activities

Typical Response

Redness and tenderness usually resolve within 1–3 hours
In sensitive skin, redness may persist up to 24 hours
A series of approximately 3–5 treatments is commonly used

FRACTIONAL ERBIUM SKIN RESURFACING (SUPERFICIAL TO INTERMEDIATE DEPTH)

Pixel Er:YAG 2940 nm Laser

Fractional Er:YAG resurfacing provides adjustable treatment depth and intensity for fine lines, wrinkles, and uneven skin texture. Treatment intensity may be progressively increased during subsequent sessions according to healing, skin response, and treatment goals.

Creates microscopic treatment zones that stimulate collagen remodeling while preserving surrounding tissue
Treatment depth and intensity are adjusted according to the skin condition and desired degree of correction
Allows treatment to progress from more superficial to intermediate-depth resurfacing as appropriate

Typical Response (varies based on treatment settings)

Immediate redness with a honeycomb pattern
Mild sunburn-like sensation and redness:
- Up to 24 hours with more superficial settings
- Approximately 2–3 days with intermediate-depth settings
Mild peeling may occur over the following several days

Topical Anesthetic

Not routinely required

Most patients return to normal activities the same day after more superficial treatment or the next day after intermediate-depth treatment, although visible redness may persist during early healing.

A series of approximately 3–5 treatments may be used, with treatment intensity adjusted according to the response to previous sessions.

SUPERFICIAL NON-FRACTIONAL ERBIUM LASER PEEL

Er:YAG 2940 nm Laser

A superficial 2940-nm Er:YAG laser peel removes a controlled thin layer of the skin surface. Unlike fractional resurfacing, treatment is applied across the entire selected treatment area rather than through microscopic treatment zones.

This lighter ablative treatment may be used to improve:

Superficial skin texture
Fine surface lines
Rough or uneven skin surface
Superficial sun-related skin changes

Treatment depth is adjusted according to the skin condition, treatment area, and desired degree of resurfacing.

Typical Response

Mild sunburn-like sensation for approximately 1–2 hours
Superficial peeling for approximately 1–2 days
Pink or sensitive skin may remain for several additional days

Topical Anesthetic

Not routinely required for superficial treatment

Most patients return to normal activities within approximately 2–3 days, depending on treatment intensity and individual skin response.

ADVANCED CO₂ FRACTIONAL LASER RESURFACING

iPixel UltraPulse CO₂

Fractional CO₂ resurfacing provides a wide range of treatment intensity and depth. At IVSI, aggressive CO₂ resurfacing is generally reserved for more advanced wrinkles, significant sun damage, deeper textural changes, or patients who have not achieved sufficient improvement with less intensive treatments.

Because deeper CO₂ resurfacing involves substantially greater downtime, it is not routinely used as a first-line resurfacing treatment.

For many patients, resurfacing begins with fractional Er:YAG treatment at more moderate settings, allowing skin response and healing to be assessed before treatment intensity is increased. Fractional CO₂ may be introduced when greater treatment depth is appropriate, and it can also be performed at moderate settings and progressively adjusted over subsequent sessions.

CO₂ Fractional Resurfacing

Creates microscopic ablative treatment zones while preserving surrounding tissue to support healing
Stimulates collagen remodeling at adjustable treatment depths
Can be performed at moderate or more intensive settings, depending on the degree of skin change being treated
Allows a staged treatment approach, with intensity adjusted over subsequent sessions according to healing and treatment response

Typical Response

Redness and swelling are expected during early healing
Crusting, peeling, and surface healing occur over the following several days
Pink or sensitive skin may persist after the initial surface healing
Recovery varies substantially with treatment depth and intensity
With more intensive fractional CO₂ resurfacing, return to normal activities may require approximately 5–7 days or longer

Anesthesia

Topical anesthetic is applied before treatment
Additional comfort measures may be used according to the treatment area and intensity

IMPORTANT CONSIDERATIONS

Laser resurfacing often requires multiple treatments, depending on the technology, treatment depth, and degree of correction desired
Treatment intensity may be adjusted or progressively increased according to healing and skin response
Greater treatment depth generally produces a more significant healing response and longer recovery period
Results vary according to skin characteristics, degree of skin aging or sun damage, treatment intensity, and individual response
The most intensive resurfacing treatment is not necessarily the most appropriate starting point
