When Treatment May Be Considered

Varicose vein treatment may be recommended when patients experience symptoms such as:

  • Leg pain or aching
  • Heaviness or fatigue in the legs

VARICOSE VEIN TREATMENT OPTIONS AT IVSI

When medically indicated, treatment may include the following minimally invasive procedures performed in our clinic:

  • Endovenous Laser Ablation (EVLA) – closes refluxing saphenous veins using laser energy under ultrasound guidance
  • Percutaneous Ablation of Perforating Veins (PAP) – treats incompetent perforator veins contributing to venous reflux
  • Ultrasound-Guided Sclerotherapy – injection treatment for refluxing or abnormal veins not suitable for laser ablation
  • Compression Sclerotherapy for Spider Veins – cosmetic treatment for small superficial veins

All procedures are performed in-office under local anesthesia without hospital admission.


TREATMENT FOLLOWS DIAGNOSIS

Varicose veins are most commonly caused by chronic venous insufficiency (CVI) — a condition in which vein valves fail and blood flows backward in the legs.

Because this process occurs beneath the skin, accurate diagnosis with venous ultrasound is essential before treatment.

When treatment is indicated, it is often performed in stages, addressing the underlying source of venous reflux rather than focusing only on visible veins.


ENDOVENOUS LASER ABLATION (EVLA)

Endovenous Laser Ablation (EVLA) is a minimally invasive treatment used to close incompetent saphenous veins responsible for venous reflux and varicose vein formation.

Using ultrasound guidance, a thin laser fiber is placed inside the affected vein through a small skin puncture. Controlled laser energy is delivered to seal the vein closed, allowing blood flow to reroute through healthier veins.

EVLA is designed to:

• Address the underlying source of venous reflux
• Reduce symptoms such as leg pain, heaviness, and swelling
• Help limit disease progression and recurrence

The procedure is performed in-office under local anesthesia and does not require hospitalization.

PERCUTANEOUS ABLATION OF PERFORATING VEINS (PAP)

Percutaneous Ablation of Perforating Veins (PAP) treats incompetent perforator veins that allow abnormal blood flow between the superficial and deep venous systems.

Under ultrasound guidance, a laser fiber is inserted through a small skin puncture into the targeted perforator vein. Laser energy is delivered to close the abnormal connection and eliminate pathologic cross-flow.

PAP may be recommended when perforator reflux contributes to persistent symptoms or advanced venous changes.

Like EVLA, the procedure is performed in-office under local anesthesia.


ULTRASOUND-GUIDED SCLEROTHERAPY (LIQUID & FOAM)

(Liquid & Foam)

Ultrasound-guided sclerotherapy involves injecting an FDA-approved sclerosant solution into refluxing or abnormal veins identified beneath the skin using real-time ultrasound guidance.

Depending on vein size and anatomy, treatment may be performed using:

  • Liquid sclerosant
  • Physician-prepared foam
  • Commercially prepared microfoam

The treated veins collapse and are gradually absorbed by the body over time.

Sclerotherapy is commonly used for:

  • Veins not suitable for EVLA
  • Residual or recurrent refluxing veins
  • Deeper varicosities not visible at the surface
  • Selected perforator veins when appropriate

It is often combined with EVLA or PAP as part of a staged treatment plan.

COSMETIC SCLEROTHERAPY FOR SPIDER & RETICULAR VEINS

Compression sclerotherapy is used to treat spider veins and reticular (blue) veins on the legs.

A medical solution is injected into small superficial veins, followed by compression to support vein closure and gradual fading.

When underlying venous insufficiency is present, deeper reflux is typically addressed first before cosmetic vein treatment.
