SYMPTOMS OF VENOUS INSUFFICIENCY

Varicose veins are most commonly caused by chronic venous insufficiency (CVI), a condition in which vein valves fail and blood flows backward in the legs.

Symptoms may include:

  • Aching or throbbing leg pain
  • Heaviness or fatigue in the legs
  • Swelling, especially later in the day
  • Bulging or enlarging veins
  • Skin discoloration near the ankles
  • Night cramps

Because venous disease develops beneath the surface, accurate diagnosis requires venous ultrasound evaluation, not visual inspection alone.

Treatment is recommended only after:

Not all varicose veins require procedural treatment. When reflux is confirmed and symptoms justify intervention, therapy is directed at the underlying source of venous reflux rather than only visible veins.

Endovenous Laser Ablation (EVLA)

EVLA treats incompetent saphenous veins that contribute to varicose vein formation.
Using ultrasound guidance, a thin laser fiber is placed inside the affected vein. Controlled energy seals the vein closed so blood flow is redirected to healthier veins.

EVLA is:

Percutaneous Ablation of Perforating Veins (PAP)

When abnormal perforator veins contribute to persistent symptoms or advanced venous disease, PAP may be recommended.

Targeted laser energy is delivered under ultrasound guidance through a small skin puncture to close the abnormal connection between the superficial and deep venous systems.


Ultrasound-Guided Sclerotherapy (LIQUID & FOAM)

Sclerotherapy treats refluxing or abnormal veins not suitable for laser ablation.

A medical solution is injected into the vein under ultrasound guidance, causing the treated vein to collapse and gradually be absorbed by the body.

Depending on vein size and anatomy, treatment may use:

  • Liquid sclerosant
  • Physician-prepared foam
  • Commercially prepared microfoam

COSMETIC SCLEROTHERAPY FOR SPIDER VEINS

Compression sclerotherapy treats spider veins and reticular (blue) veins on the legs.

A medical solution is injected into small surface veins followed by compression to support vein closure and gradual fading.

When underlying venous insufficiency is present, deeper reflux is typically treated first before cosmetic refinement.
