Not Sure What Type of Skin Growth It Is?

Many patients are unsure whether a skin change represents a mole, skin tag, keratosis, vascular lesion, or another type of benign skin growth.

Skin lesions evaluated in dermatologic surgery may appear as:

• pigmented moles or dark spots
• rough or wart-like growths
• small soft skin tags
• red vascular lesions
• raised or thickened areas of skin

A clinical examination helps determine whether the lesion requires monitoring, biopsy, or removal.


Skin Lesion Removal Methods

When treatment is indicated, several procedural options may be considered depending on lesion type, location, and diagnostic requirements.

Therapeutic Skin Procedures

Diagnostic Biopsy Procedures

Biopsy procedures are performed to establish or confirm the diagnosis of a skin lesion when clinical examination alone is not sufficient.

All tissue samples are sent for histopathologic examination.


Surgical Excision

Surgical excision with layered suture closure removes a lesion in its entirety with margins appropriate to the clinical diagnosis.

This method is commonly used when:

• complete removal is necessary
• diagnostic certainty is required
• malignancy must be excluded or treated


CO₂ Laser–Assisted Evaporation / Destruction

The ablative surgical CO₂ laser removes tissue through controlled vaporization.

This technique is performed for selected benign or premalignant lesions, including:

• actinic keratoses
• inflamed seborrheic keratoses
• selected superficial lesions

Laser-assisted procedures allow precise tissue removal with good control of bleeding.


Laser Treatment of Vascular Growths

Non-ablative Class IV medical lasers are used for selected vascular skin lesions.

These lasers selectively target hemoglobin within abnormal blood vessels, resulting in shrinkage of the lesion.

Examples include:

• pyogenic granuloma
• hemangioma
• selected vascular facial lesions


Cosmetic Removal of Benign Skin Lesions

In selected cases, removal of benign skin lesions may be performed to improve appearance or comfort.

Procedural options include:


Surgical Excision

Surgical excision with layered suture closure or reconstructive repair may be performed for larger or deeper lesions when complete removal is desired, including certain facial moles.


CO₂ Laser–Assisted Shaving and Ablation

Laser-assisted shaving removes the raised portion of a lesion while smoothing the surrounding skin surface.

This technique is commonly performed in cosmetically sensitive areas where contour preservation is important.


CO₂ Laser Destruction / Evaporation

Laser evaporation removes small superficial benign lesions.

Examples include:

• skin tag removal on eyelids and body
• small seborrheic keratoses on the face, neck, or chest
• selected superficial lesions on other areas of the body


Pigment Lightening

Pigment-targeting laser treatments lighten dark pigmentation in selected flat benign lesions.

These treatments affect pigment only and do not remove the full depth of the lesion.


Choosing the Appropriate Removal Method

A focused skin examination allows evaluation of specific lesions or areas of concern based on patient preference and clinical findings.

Digital dermatoscopy is commonly used when evaluating melanocytic nevi and other pigmented lesions.

The selected treatment method depends on whether the primary goal is:

• surface contour improvement
