Varicose vein disease is progressive. It does not resolve on its own and typically worsens over time. Effective treatment requires accurate diagnosis, thoughtful staging, and correction of underlying venous reflux.
Understanding Varicose Vein Disease
Varicose vein disease — also known as chronic venous insufficiency (CVI) — occurs when vein valves fail and blood flows backward in the superficial veins of the legs.
CVI cannot be reversed with medication. There is no treatment that repairs damaged vein valves. While certain measures may temporarily reduce symptoms, they do not correct the underlying venous dysfunction.
Definitive treatment focuses on closing or removing malfunctioning superficial veins that are no longer functioning properly.
Types of Varicose Vein Treatment
Treatment strategies fall into two primary categories:
1. Conservative Management
Conservative therapy includes non-invasive measures that may help reduce symptoms and improve functional comfort, such as:
- Graduated compression stockings
- Regular walking and physical activity
- Weight management and lifestyle modification
While conservative management may reduce discomfort, swelling, or heaviness, it does not eliminate diseased veins or correct venous reflux.
Conservative therapy may be recommended as an initial step, as part of insurance requirements, or when procedural treatment is not yet indicated.
2. Corrective (Procedural) Treatment
Corrective treatment involves minimally invasive procedures designed to close or remove malfunctioning superficial veins responsible for venous reflux.
These may include:
- Endovenous Laser Ablation (EVLA)
- Percutaneous Ablation of Perforating Veins (PAP)
- Ultrasound-Guided Sclerotherapy
- Ambulatory Phlebectomy
When medically indicated based on clinical findings and venous ultrasound evaluation, treatment is directed at the underlying source of reflux rather than only visible veins.
Optimal outcomes are often achieved through a staged combination of conservative and corrective treatment strategies, guided by the severity and pattern of reflux, the procedures performed, and patient-specific factors including comfort and recovery considerations.

UNDERSTANDING THE TREATMENT PROCESS
Varicose vein treatment is structured and staged.
Correction of the primary source of reflux is performed first, followed by reassessment and treatment of residual veins when indicated.
Care typically includes:
- Closure of the primary refluxing (axial) veins
- Follow-up ultrasound evaluation
- Treatment of remaining branch or residual varicosities as needed
All currently available treatments work by closing or removing abnormal superficial veins. There are no procedures that repair or rebuild damaged vein valves.
The goals of treatment are to:
- Relieve symptoms
- Improve circulation efficiency
- Reduce venous hypertension
- Slow progression of chronic venous insufficiency
Treatment is not considered a cure for CVI, but rather a structured intervention designed to improve function and reduce disease-related complications.
Why Only Superficial Veins Are Treated
Many patients worry that closing veins could impair circulation. In reality, treatment targets only superficial veins, which normally carry a small percentage of blood from the legs.
The deep venous system, responsible for the majority of leg circulation, is never treated and remains fully intact.
Superficial varicose veins often have damaged valves and allow blood to flow backward, contributing to pain, swelling, and skin changes. Closing these nonfunctional veins redirects blood into healthier pathways, supporting more efficient circulation while preserving the veins essential for normal blood flow.

Endovenous Laser Ablation (EVLA)
A minimally invasive, ultrasound-guided procedure used to close refluxing saphenous veins. -
Percutaneous Ablation of Perforating Veins (PAP)
Recommended when incompetent perforator veins contribute to persistent symptoms or advanced venous changes. Using ultrasound guidance, targeted laser energy is delivered through a small skin puncture to close the abnormal connection between the superficial and deep venous systems. -
Ultrasound-Guided Sclerotherapy (Liquid & Foam)
Injection-based treatment for deeper, residual, or anatomically unsuitable varicose veins. -
Compression Sclerotherapy
Treatment for reticular veins and selected spider veins. -
Ambulatory Phlebectomy
Removal of large, bulging surface varicose veins when appropriate. -
Cosmetic Sclerotherapy of Spider Veins
Treatment to improve the appearance of visible spider or small reticular veins when medically appropriate.
